Academic Forms

Note:
The Graduate School has recently switched from paper to online forms for most academic purposes. Please log onto CAESAR to fill out necessary forms, and submit them online. After you submit the forms online, and after you have completed your proposal or defense (where applicable), your paperwork will be approved first by the department and then by the Graduate School online.

Most other forms are in pdf format. In order to open them, you need to have the latest version of Acrobat Reader installed. Download the application for free here. If you edit the forms in Adobe Reader, you will not be able to save a copy, however you will be able to print the form with your additions.

• Students must fill out the Master's Completion form on CAESAR and submit it online at least two days prior to the MS defense.
• Send Allison Sillers an email upon submission of the form online.

Qualifying Exams

• Students must fill out the PhD Prospectus form on CAESAR and submit it online at least two days prior to the proposal
• Send Allison Sillers an email upon submission of the form online.

4th Year Review

• Form for the 4th Year Review (PDF, 16 KB)

PhD Completion Paperwork

• Students must fill out the PhD Defense form on CAESAR and submit it online at least two days prior to the defense
• Send Allison Sillers an email upon submission of the form online.
• Change of Grade forms (triplicate form, should be taken care of by TGS or Allison) - you should not need to take care of this!
• Survey of Earned Doctorates - please print and fill out by hand, and return directly to the Graduate School
